Sample Contract:

ICANL APPLICATION SERVICES:

Associates in Medical Physics, LLC (AMP) agrees to supply the following services:


• AMP will acquire data needed to prepare the ICANL (Intersocietal Commission on the Accreditation of Nuclear Medicine Laboratories) application. An initial site visit will be required prior to the start of preparation of the application.

• Assist with the development of policies, procedures and protocols to meet the Standards of the ICANL.

• Advise in selection of equipment and supplies to meet ICANL Standards.

• Provide consulting and reference assistance for instituting ICANL’s Standards into the nuclear medicine program. Site visits may be necessary to assist in implementing the Standards.

• Limited assistance in establishment of a radiological health program and record system in accordance with ICANL Standards.


CONSULTING SERVICES:

Associates in Medical Physics, LLC (AMP) agrees to supply the following services:

AMP will visit your facility semi-annually to:

a. Offer consultation on problems pertaining to your ICANL Program.

b. Confirm that the medical director and technical director are meeting required responsibilities.

c. Review radioactive materials license for accuracy to meet ICANL standards.

d. Review continuing education records of medical and technical staff to assure adequacy of standards.

e. Review quality assurance evaluations on scintillation camera(s), uptake probe and well detector units, as time and patient schedules permit.

f. Review clinical protocols for accuracy of daily practice. This will include observation of the technical staff.

g. Sample patient reports for conformity of reporting by all interpreters.

h. Administrative, technical and interpretive quality assessment will be evaluated. This will include the review of questionnaires offered to referring physicians and patient satisfaction forms, technical reproducibility and medical staff peer review records.
